

Provides professional nursing care, health assessments, and clinical oversight for residents.
Coordinates medical care with physicians, NPs, therapists, and care staff; develops and monitors care plans.
Delegates tasks to unlicensed staff within state rules; ensures medication safety and adherence to orders.
Serves as a clinical lead within the care team and may supervise LPNs/LVNs, CNAs, and DSPs per policy.
Education & licensure
Active Texas RN license from the Texas Board of Nursing (BON), in good standing.
Graduation from an accredited Registered Nurse program.
Eligibility for licensure verification (e.g., license verification for out-of-state hires).
Experience
Prior experience in inpatient, outpatient, community-based, IDD/HCBS, or residential care settings is preferred.
Experience with care planning, medication administration, and clinical assessments is typical.
Background checks
Successful completion of fingerprint/background checks per HHSC/employer policy.
Continuing education
Ongoing compliance with BON CE requirements; site-specific nursing training may be required (infection control, medication policies, rights protections).
Scope of practice
Practice under the Texas Nurse Practice Act and BON rules.
Authorized to administer medications and therapies as ordered, within facility policies and delegation rules.
Develop, implement, and evaluate individual resident care plans in collaboration with the care team.
Supervision & delegation
Supervise or coordinate with LPNs/LVNs, CNAs, and DSPs as allowed by state law and agency policy.
Ensure appropriate nurse delegation, task assignments, and supervision levels.
Documentation
Maintain accurate nursing documentation: care plans, progress notes, MARs/medication records, incident reports, and health assessments.
Ensure documentation supports regulatory compliance and quality of care.
Health status & immunizations
Meet employer and HHSC health requirements; immunizations as required.
Maintain CPR/BLS certification and any site-specific emergency training.
Patient rights & safety
Uphold resident rights, safety, and privacy; report abuse/neglect per policy.
Infection control
Adhere to infection control policies, standard precautions, and PPE usage.
Onboarding
Orientation on agency policies, TAC/HHSC requirements, resident rights, documentation standards, and medication policies.
Clinical training
Medication administration policies, wound care, vital signs monitoring, chronic disease management, and emergency procedures.
Training on care coordination, data collection, and use of electronic health/records systems.
Professional development
Opportunities for continuing education and competency validation (clinical skills refreshers, etc.).
Conduct health assessments, triage health concerns, and coordinate medical care.
Administer medications and treatments as ordered; monitor for adverse effects.
Develop and update care plans; review and adjust with the care team.
Document health status, progress, and incidents; maintain HIPAA-compliant records.
Collaborate with physicians, RNs in the team, therapists, case managers, and families.
Provide staff training and supervision related to clinical tasks and resident health.
