

Provide hands-on daily living support to residents (ADLs, mobility, non-clinical care) and supervision to ensure safety.
Support community integration, rights protection, and resident well-being.
Work under the supervision of a House Supervisor, RN/LPN, or program director; collaborate with nursing, therapy, case management, and family/facilitators.
Education: 18 years of age or older; high school diploma or GED preferred.
Experience: Prior direct care or HCBS/residential care experience is a plus but not always required.
Background checks: Successful completion of fingerprint/background checks per HHSC/employer policy.
Licensing/credentials: No professional license is typically required; some programs may value CNA/medical-related credentials or medication-aided training if applicable.
Driving (if required): Valid driver’s license and clean driving record if transporting residents or supplies.
Orientation: Agency-specific onboarding on policies, resident rights, confidentiality, and TAC/HHSC requirements.
Core trainings:
Person-centered planning basics
Abuse/neglect reporting and safeguarding residents
Infection control, hand hygiene, and PPE
Safety, crisis de-escalation, and de-escalation techniques (CPI or similar)
Rights of residents and protection measures
Basic first aid/CPR (likely required or strongly encouraged)
Ongoing competencies: Regular in-service trainings and skill validations (ADL support, documentation, incident reporting).
Health status: Meet employer and HHSC health requirements; required immunizations as applicable.
Safety culture: Adhere to safety policies; report hazards and incidents promptly.
Infection control: Follow facility cleaning/disinfection protocols and PPE guidelines.
Rights & dignity: Respect resident rights; maintain confidentiality and professional boundaries.
Assist residents with activities of daily living (bathing, dressing, grooming, toileting) and mobility.
Support meal preparation, feeding assistance, and nutritional monitoring as directed.
Assist with medication reminders or administration only if trained and authorized by policy (often via a separate medication-aided path or under supervision).
Help with transportation or accompany residents to appointments if permitted.
Monitor health status, report changes to nurses or supervisors, and document observations.
Document care provided, incidents, and daily activities; maintain shift handoffs.
Engage residents in social, recreational, and community activities as appropriate.
Report to House Supervisor or Program Supervisor; work with DSP teams to ensure consistency of care.
Collaborate with nursing, therapy, case management, and family members to support resident goals.
Follow supervision and escalation protocols for incidents, safety concerns, or health changes.
Maintain accurate records of care provided, observations, and incident reports per agency policy.
Ensure adherence to TAC/HHSC requirements for the service type.
Protect resident privacy and comply with HIPAA and data security policies.
